OSTEOPATHIC RESIDENCY APPLICATION PROCESS Office of Medical Education To complete an internship application, you must register with ERAS. For levels above PGY-1, directly contact the program you are interested in. We offer combined Internship/Residency Training Programs in Family Medicine and General Surgery as well as an Obstetrics and Gynecology Track Program. APPLICATION AND ADMISSION CHECKLIST All appropriate documentation MUST be received via ERAS for internship / special emphasis /specialty track programs. For advanced training, in order for your application to be processed and considered for interview: 1. Completed application 2. Three letters of recommendation from physicians 3. Dean's letter from your medical school 4. Copies of all board scores, (USMLE, COMLEX Parts I, II, III) 5. Copy of medical school diploma or a certificate of scheduled completion 6. Copy of all medical school transcripts Letter(s) of recommendation from any prior Residency Program Directors  7. Copy of all licenses (if applicable) 8. Copy of current alien registration or visa (if not a U.S. citizen)
