Who Will Speak For You?
In New York State, there exists a law called the New York Health Care Proxy law. This law allows you to appoint someone you trust, such as a family member or a close friend, to decide about medical treatment if you lose the ability to decide for yourself.

Why is a Health Care Proxy Important?
If you ever became unable to communicate with the doctors, or any other medical staff involved in your care without a health care proxy, family members would not be allowed to make decisions about your treatment; even if they know what you would choose. Completing a health care proxy empowers a trusted individual to carry out your wishes with regard to your medical treatment.

Where Can a Health Care Proxy be Obtained?
Health care proxy forms are available through the Patient Advocate Department at Wyckoff Heights Medical Center.
